The information contained in this application is correct to the best of my knowledge. Any omission or misrepresentation in the application process will cause me to be denied employment/lease opportunity. If any omission or misrepresentation is discovered after I have been employed/leased by Stage Call, I will be released from employment/lease and be responsible for my own transportation home.
I hereby authorize Stage Call and its designated agents and representatives to conduct a comprehensive review of my background causing a consumer report and/or an investigative consumer report to be generated for employment and/or contract lease. I understand that the scope of the consumer report/investigative consumer report may include, but is not limited to the following areas: verification of social security number; credit reports; current and previous residences; employment history; education background; character references; drug testing; civil and criminal history records from any criminal justice agency in any or all federal, state, county jurisdictions; driving records; birth records; and any other public records.
I further consent to the furnishing of such information described above to third parties if necessary to Stage Call fulfilling its official responsibilities regarding my application for certification. I hereby release any individual, agency and institution from any and all liability for damages of whatever kind, which may result from complying or attempting to comply with this authorization and request to release information. I may be contacted at the address indicated above should there be any questions as to the validity or authenticity of this release.
I further authorize any individual, company, firm, corporation, or public agency to divulge any and all information, verbal or written, pertaining to me, to Stage Call or its agents. I further authorize the complete release of any records or data pertaining to me which the individual, company, firm, corporation, or public agency may have, to include information or data received from other sources. Stage Call and its designated agents and representatives shall maintain all information received from this authorization in a confidential manner in order to protect the applicants personal information, including, but not limited to, addresses, social security numbers, and dates of birth.
In connection with your application for lease contract/employment with Stage Call Corporation, it may obtain one or more reports regarding your driving, and safety inspection history from the Federal Motor Carrier Safety Administration (FMCSA).
When the application for lease contract/employment is submitted in person, if Stage Call Corporation uses any information it obtains from FMCSA in a decision to not lease contract/hire you or to make any other adverse lease contract/employment decision regarding you, Stage Call Corporation will provide you with a copy of the report upon which its decision was based and a written summary of your rights under the Fair Credit Reporting Act before taking any final adverse action. If any final adverse action is taken against you based upon your driving history or safety report, Stage Call Corporation will notify you that the action has been taken and that the action was based in part or in whole on this report.
When the application for lease contract/employment is submitted by mail, telephone, computer, or other similar means, if Stage Call Corporation uses any information it obtains from FMCSA in a decision to not hire you or to make any other adverse lease contract/employment decision regarding you, Stage Call Corporation must provide you within three (3) business days of taking adverse action oral, written or electronic notification: that adverse action has been taken based in whole or in part on information obtained from FMCSA; the name, address, and the toll free telephone number of FMCSA; that the FMCSA did not make the decision to take the adverse action and is unable to provide you the specific reasons why the adverse action was taken; and that you may, upon providing proper identification, request a free copy of the report and may dispute with the FMCSA, the accuracy or completeness of any information or report. If you request a copy of a driver record from Stage Call Corporation who procured the report, then, within three (3) business days of receiving your request, together with proper identification, Stage Call Corporation must send or provide to you a copy of your report and a summary of your rights under the Fair Credit Reporting Act.
Stage Call Corporation cannot obtain background reports from FMCSA unless you consent in writing.

I authorize Stage Call Corporation to access the FMCSA Pre-Employment Screening Program (PSP) system to seek information regarding my commercial driving safety record and information regarding my safety inspection history. I understand that I am consenting to the release of safety performance information including crash data from the previous five (5) years and inspection history from the previous three (3) years. I understand and acknowledge that this release of information may assist Stage Call Corporation to make a determination regarding my suitability as a lease contractor/employee.  Â
I further understand that neither Stage Call Corporation nor the FMCSA contractor supplying the crash and safety information has the capability to correct any safety data that appears to be incorrect. I understand I may challenge the accuracy of the data by submitting a request to: https://dataqs.fmcsa.dot.gov.  If I am challenging crash or inspection information reported by a State, FMCSA cannot change or correct this data. I understand my request will be forwarded by the DataQs system to the appropriate State for adjudication.
Please note: Any crash or inspection in which you were involved will display on your PSP report. Since the PSP report does not report, or assign, or imply fault, it will include all Commercial Motor Vehicle (CMV) crashes where you were a driver or co-driver and where those crashes were reported to FMCSA, regardless of fault. Similarly, all inspections, with or without violations, appear on the PSP report. State citations associates with FMCSR violations that have been adjudicated by a court of law will also appear, and remain, on a PSP report.
I acknowledge by my signature that a pre-employment or pre-contract drug screen test will be done if I am offered a position as a driver. This pre-employment/pre-contract drug screen test result must have a negative result for me to be employed or contract leased as a driver by Stage Call. I also acknowledge that I may be asked to take a physical examination as part of my employment or contract lease requirements.
Applicants who have previous Department of Transportation regulated employment history in the preceding three (3) years, and wish to review previous employer-provided investigative information, must submit a written request to the prospective employer, which may be done at any time, including when applying, or as late as 30 days after being employed or being notified of denial of employment. The prospective employer must provide this information to the applicant within five (5) business days of receiving the written request. If the prospective employer has not yet received the requested information from the previous employer(s), then the five (5) business days deadline will begin when the prospective employer receives the requested safety performance history information. If the applicant has not arranged to pick up or receive the requested records within thirty (30) days of the prospective employer making them available, the prospective motor carrier may consider the applicant to have waived his/her request to review the records.
I understand that information I provide regarding current and/or previous employers may be used, and those employer(s) will be contacted for the purpose of investigating my safety performance history as required by 49 CFR 391.23(d) and (e).
I understand that I have the right to:
- Review information provided by current/previous employer(s);
- Have errors in the information corrected by previous employer(s) and for those previous employer(s)s to re-send the corrected information to the prospective employer; and
- Have a rebuttal statement attached to the alleged erroneous information, if the previous employer(s) and I cannot agree on the accuracy of the information.
